Flight recorder evidence suggests the crew of an Amazon Prime Air cargo aircraft made a late attempt to abandon its landing after the Boeing 767 was already on the runway, US investigators said on Tuesday.
The 767, operated by Miami-based 21 Air, left the runway at Miami International Airport on Sunday, travelling roughly 1,300 feet (396 m) beyond the paved surface before hitting two vehicles. Five people were killed and another five injured.
The National Transportation Safety Board said its initial examination of the flight data recorder had identified a sequence of power and braking changes that could indicate the pilots were trying to execute a go-around.
According to the NTSB, the nose gear and right main gear contacted the runway around 30 seconds before the recorder stopped, with the aircraft travelling at 158 knots (181 mph). The left main gear touched down approximately 11 seconds later, at 134 knots.
Engine power was subsequently increased and the brakes released. The combination of those actions was consistent with the use of go-around thrust, investigators said, raising the possibility that the crew attempted to get airborne again despite the aircraft already being on the runway.
That decision appears to have been short-lived. About 11 seconds before the end of the recorded data, power was brought back to idle and braking resumed. Ground speed had fallen to 65 knots by the time the recording ended.
The data examined so far also contains no indication that the aircraft’s speed brakes or thrust reversers were deployed during the landing roll, the NTSB said.
The sequence will now form part of a broader investigation into why the aircraft was unable to stop within the available runway and what prompted the apparent late go-around attempt.
NTSB Chair Jennifer Homendy said investigators planned to interview both pilots on Wednesday. Details from the cockpit voice recorder are expected to provide further insight into what was happening in the flight deck during the critical final moments.
A team of 32 NTSB investigators is working at Miami International Airport. One of the two runways closed following the accident was reopened for departures towards the east on Tuesday evening, helping to reduce the operational impact of the investigation.
The aircraft had already completed two sectors that Sunday before the crash. It flew from Cincinnati to Miami and subsequently operated a Miami-San Juan flight before returning from Puerto Rico for the accident flight, which occurred at 1:53 p.m. EDT (1753 GMT).
The NTSB said the captain, aged 55, had obtained his Boeing 767 type rating in May and had accumulated 7,145 hours of flight time. The 37-year-old first officer received his 767 type rating in April 2025 and had 2,655 hours of flying experience.
